A team of experienced Respiratory Specialists

  • Respiratory Consultants
  • Respiratory Nurse Specialists/Advanced Practitioners
  • A Service Co-ordinator
  • Digital applications (apps) are used to monitor your COPD symptoms within your own home.
  • The specialist team monitor your symptoms remotely using the apps. They will respond to any changes either by seeing you face to face or completing a telephone review as appropriate.

 

  • You are asked to complete a daily questionnaire via the app designed to monitor your COPD symptoms.
  • Exacerbations are identified early through a traffic light warning/alert system.
  • Hand-held spirometry readings may be requested by the team to support your care.
  • Specialist support is provided either through face-to face contact or telephone consultation according to your changing COPD symptoms.
  • Your ongoing care will be discussed with an allocated Respiratory Consultant as required.
  • We work alongside other Health Care Professionals who may offer some additional benefits to your ongoing care.
